  • Physical activity;

  • Preventing, detecting and treating complications; and
  • Goal setting and problem solving.

In addition to providing you with the necessary skills to manage your diabetes, we provide a support network that assists you in keeping track of necessary appointments to further manage this disease and help prevent some of the acute and chronic complications that can occur if not adequately controlled.

Video Visits: From our Home to Yours

To help keep our patients safe during the COVID-19 outbreak, we will be using video visits whenever possible to meet your healthcare needs. If you receive a prescription at the end of your virtual visit, it can be sent directly to a pharmacy most convenient for you. Your provider will help to determine when an in-person visit would be best.
